Privacy Policy

How Health Guard collects, uses and protects information on this website, and your California privacy rights.

Effective date: November 6, 2026

Health Guard Inc. (“Health Guard,” “we,” “us” or “our”) operates an adult day health care center at 5648 Vineland Avenue, North Hollywood, California 91601, and this website, healthguardinc.com. This Privacy Policy explains what information we collect through this website, how we use and share it, and the choices and rights you have, including rights under California law.

1. Scope of this policy

This policy applies to information collected through healthguardinc.com, including our contact, facility tour and call-back forms. It does not cover the health information of people enrolled in our program. Participant health information is protected under federal and California health privacy laws, including HIPAA and the California Confidentiality of Medical Information Act, and is described in our Notice of Privacy Practices, which is available at our center or by calling us at (818) 760-7727.

2. Information we collect

Information you give us

When you fill out a form on our website, we collect the information you choose to provide, such as:

  • Your name, phone number and email address
  • Your preferred date or time for a facility tour
  • The message you write to us, and which form you used

Please do not include medical details, insurance numbers or other sensitive health information in our website forms. If you need to share health information, please call us at (818) 760-7727.

Information collected automatically

When you visit our website, we and our service providers automatically collect certain information, such as your IP address, browser and device type, the pages you view, the date and time of your visit and the website that referred you. We collect this through server logs, cookies and similar technologies, described in section 6.

3. How we use your information

  • To respond to your questions, call you back and schedule facility tours
  • To send you an email confirmation when you submit a form with your email address
  • To understand how visitors use our website and improve it
  • To keep our website secure and prevent spam, fraud and misuse
  • To comply with the law and protect our rights

4. How we share your information

We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We share information only as needed with:

  • Service providers that help us run this website, such as our web hosting company, our security and content delivery provider (Cloudflare), our email delivery service (Brevo), Google (Google Analytics, Google Maps and the email accounts that receive form messages) and the agency that maintains our website. These providers may use the information only to perform services for us.
  • Legal and safety purposes, when required by law, court order or government request, or to protect the rights, property or safety of Health Guard, our members or others.
  • Business transfers, if Health Guard is involved in a merger, sale or similar transaction, subject to this policy.

5. Website forms and email

Form submissions are emailed to our staff and stored securely in our website system so we can follow up with you. If you give us your email address, you will receive an automatic confirmation. We do not add you to marketing email lists from these forms.

6. Cookies, analytics and maps

  • Google Analytics: we use Google Analytics to measure website traffic, such as how many people visit a page and whether a form was submitted. Google Analytics uses cookies and collects information such as your IP address and device type. We keep analytics data for up to 14 months. You can opt out with the Google Analytics opt-out browser add-on. Learn more about how Google uses information from sites that use its services.
  • Google Maps: some pages include a Google Map showing our location. Google may collect information when the map loads, under Google’s Privacy Policy.
  • Essential cookies: our website and security provider may use cookies that are needed for the site to work and stay secure.

We do not use advertising or retargeting cookies. You can block or delete cookies in your browser settings, although some parts of the website may not work as intended.

Do Not Track and Global Privacy Control: our website does not currently respond to browser “Do Not Track” signals. Because we do not sell or share personal information, there is nothing to opt out of; if your browser sends a Global Privacy Control signal, we treat it as a request to opt out of any sale or sharing.

7. How long we keep information

We keep website form information only as long as needed to respond to you and for normal business records, generally no longer than three years, unless a longer period is required by law or you become a member of our program. Analytics data is kept for up to 14 months.

8. How we protect information

We use reasonable administrative, technical and physical safeguards, including encrypted (HTTPS) connections, secured accounts and limited access. No website or email system is completely secure, so please avoid sending sensitive information through online forms or email.

9. Your California privacy rights

If you are a California resident, you may have the following rights, to the extent the California Consumer Privacy Act (CCPA), as amended by the California Privacy Rights Act, applies to Health Guard:

  • Right to know what personal information we collect, use and disclose, and to receive a copy
  • Right to delete personal information we collected from you, subject to legal exceptions
  • Right to correct inaccurate personal information
  • Right to opt out of the sale or sharing of personal information (we do not sell or share it)
  • Right to limit the use of sensitive personal information (we do not use it for purposes that require this right)
  • Right to non-discrimination for exercising your rights

California “Shine the Light” (Civil Code section 1798.83): we do not share personal information with third parties for their own direct marketing purposes.

We will verify your identity before responding, for example by confirming the contact details you gave us. You may use an authorized agent with your signed permission. We aim to respond within 45 days, as allowed by law.

10. Children’s privacy

Our website is intended for adults and is not directed to children under 16. We do not knowingly collect personal information from children under 16. If you believe a child has given us information, please contact us and we will delete it.

Our website links to other sites, such as government agencies, health plans and directories. We are not responsible for their privacy practices, so please review their policies.
